Since 1983, our expert team has worked to reduce the risk of isolation by connecting people with possibilities across the Monadnock Region. We believe every person, regardless of ability, has the right to make a meaningful life. This belief inspires all we do for those we serve.

You may be here because of a new diagnosis that has altered your family’s lives. Or you’re in a new place with different care systems than you’ve known before. We are here to provide support as you navigate these changes, and to make what can be a challenging experience as easy as possible.
Here, you will find services and supports for individuals from birth through adulthood; connections to a wealth of local, regional, and state resources; news and events geared toward building a vibrant community; and more. It all starts with our intake form, which is the first step in your journey with us. Again, welcome–and if you have any questions, just let us know. We’re here to both enrich and simplify your life.
If you are seeking services for individuals with developmental disabilities, your first step is right here. Please fill out this Intake Form and Referral Form to start the process.
We can help determine eligibility for your family member. Call or email the MDS Intake Coordinator who can lead you through the eligibility process, and also help in setting up services once eligibility is determined. Learn more about who we serve here.
