Children's Individual Services works with the NH Division of Children, Youth and Families to provide quality foster care for children and adolescents with developmental disabilities and who are at risk of placement in an institutional setting

Through a comprehensive array of services, we provide supports that address the whole person, ensuring a safe, healthy and nurturing quality of life for every child we serve.

 

We get involved. . .
We place children with developmental and/or emotional disabilities in long-term residential foster care homes. A coordinator, responsible for managing the many aspects of the child's care, is assigned to each family. Coordinators assist with:

Coordinators have small caseloads allowing for frequent and meaningful contact with each family. Home visits occur at least once a month in addition to bi-weekly visits in other settings such as at school, with the child's biological family, physician's office or other health care provider, or elsewhere in the community. Twenty-four hour support and crisis intervention are always available.

It's about the kids
A child-focused approach facilitates supports and services geared to each child's unique needs. Gentle-Approach training is available to the child's service team to insure consistent and effective interventions for behavioral and/or emotional issues.

Quality services for quality of life
Everyone associated with the child - family members, support staff, coordinators and other service providers - are trained by a nurse consultant assigned to each service team.

Team members are licensed to give medications to children who are unable to take medication themselves.

On-site medication reviews are conducted monthly by a nurse to ensure medications are properly ordered and dispensed.

Hiring, training and supervision of support staff is managed by the coordinator to ensure ongoing quality of care.
