Children's Service Coordination is a resource to families who have a child with a developmental disability or chronic health condition. The services provided are initiated by each family and they are unique to their specific needs. The focus of Children's Service Coordination is to assist the family in accessing services for the individual with the disability or chronic health condition. Our goal is to be a resource for educating and empowering each family. This is achieved by partnering with a Children's Service Coordinator.

Children's Service Coordination includes:

* Information and referral to community services
* Assistance in planning for transitions
* Access to training
* Assistance with future planning
* Use of Lending Library
* Information about acquiring benefits
* Parent to Parent matches
* Referral to appropriate support programs
* Assistance to families with the special education process

 

Partners in Health

Partners in Health (PIH) is a community-based family support program designed to address the needs of children with chronic health conditions and their families. The PIH coordinator works with schools, medical providers, social services and other community organizations to create a setting that will enhance a family's quality of life.

The Family Support Council

The Family Support Council is an advisory group to the Area Agency. The Council is made up of parents and family members of children and adults with developmental disabilities. The Council provides the direction and focus for the Children's Services program and actively advocates for families in the Monadnock Region. The Council supports the role of families in the decision-making process regarding the design and implementation of the supports and services available to individuals with developmental disabilities and their families. Monthly meetings are open to all families and everyone is encouraged to attend.

Support Groups

Parent support for parents of children with chronic health conditions or special needs. Meets the first Tuesday of every month from 7:00 to 8:30 at MDS, 121 Railroad Street, Keene. Call 352-1304, for more information.

Parents Anonymous meets the first and third Thursday of every month from 5:30 to 7:00 at 24 Vernon Street, Keene. Call 355-3082 for more information.

Parent support group for parents of children with special needs meets on Tuesdays at 7:00 at the Grapevine in Antrim. Call 588-2620 for more information.

Single parent support group meets the 4th Wednesday on the month from 6:30 to 8:00 at 24 Vernon Street, Keene. Call 355-3082 for more information.

Support for parents of children with Asperger's Syndrome. Meets the third Monday monthly at Keene State College. Call 358-2517 for more information.

CH.A.D.D. Children with ADD support and information. Meets monthly. For information, call Joanne Matthers 357-0479.
